Team GRITT/KC Sports Factory LLC Participation Waiver for Players

In consideration of being allowed to participate in any way in the GRITT Basketball Lab /KC SportsFactory LLC program the undersigned acknowledges and agrees that:

  1. I accept the responsibility to ensure that I am medically and physically fit before entering into competition.
  2. However, the risk of injury and/or illness from the activities related to the sports involved in the program is still significant including but is not limited to ankle sprains, concussions, torn ligaments, the possibility of paralysis and death.
  3. I understand that by attending events and practices that there is a risk of contracting communicable disease, including but not limited to COVID-19. I understand that there are policies in place set by GRITT Basketball Lab to ensure reduced transmission as outlined by the CDC. While these are in place, the risk is still not zero and I assume such risk and do not hold Team GRITT or any other KC Sports Factory LLC entity liable for any such illnesses come into contact with or come into close proximity with while in their facilities or at their events
  4. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual, significant hazard during my presence or participation, I will remove myself from participation and bring such hazard to the attention of the nearest official immediately; and,
  5. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless GRITT Basketball Lab/ KC SportsFactory LLC its officers, officials, agents and/or employees, other participants, sponsoring agencies, directors, advertisers, and, if applicable, owners and lessors of the premises involved with the disciplines from any actions, claims, liabilities, losses, costs, or expenses which might arise from such injury or accidents.

GRITT Basketball Lab/KC Sports Factory LLC Waiver & Code of Conduct for Spectators

  1. I understand that by attending events and practices that there is a risk of contracting communicable disease, including but not limited to COVID-19. I understand that there are policies in place set by GRITT Basketball Lab to ensure reduced transmission as outlined by the CDC. While these are in place, the risk is still not zero and I assume such risk and do not hold Team GRITT or any other KC Sports Factory LLC entity liable for any such illnesses I come into contact with or come into close proximity with while in their facilities or at their events
  2. I promise to be a model of behavior for the kids on the team by being encouraging to the athletes participating as well as the refs and coaches at the event
  3. I will accept my role as a parent at both games and practices and know to not overstep the boundaries of a parent by interjecting or coaching the team while they are actively playing
  4. I will remember that the athletes participating are expected to have fun and enjoy the game while learning the fundamentals and I will not overemphasize a win or a loss to prioritize over enjoyment of the game
  5. At all time we must conduct ourselves in a highly respectable manner. No bad mouthing the refs, coaches, other players, or arguing with the opposite team fans. At all times parent/ child must take the highroad.
  6. I hereby release and hold harmless GRITT Basketball Lab/KC SportsFactory LLC and all associated parties involved with the disciplines from any actions, claims, liabilities, losses, costs, or expenses which might arise from such injury or accident

COVID policy:

All GRITT parents are required to sign our COVID waiver for their child and self-prior to the first practice. All GRITT athletes are expected to wear a mask at all times during practice. Hand sanitizer is to be used frequently during practice, especially at the beginning, all water breaks and at the end of practice. Hand sanitizer will be provided by coaches. During games, athletes are expected to wear a mask and social distance while on the bench. Game to game guidelines is subject to the governing league or tournament programs guidelines as well. All parents are required to report any possible COVID exposure, positive COVID test or suggested quarantine times to coach and program director as soon as possible. If experiencing any of the symptoms below, please notify coach and program director ASAP: cough, fever > 100.4, shortness of breath, chills, sore throat, fatigue, body aches, new loss of taste or smell and nasal congestion.
